What it is
A real BAA. Not compliance theatre.
Most consumer-grade transcription apps prohibit clinical use in their terms but quietly look the other way. We don’t. Loqui’s base product is not HIPAA-compliant and we actively friction-test clinical use to keep it that way.
The Clinical tier is a separate plan with executed Business Associate Agreements with Loqui and every subprocessor that touches PHI: AssemblyAI’s HIPAA tier for transcription, Anthropic’s BAA-arranged Claude endpoint for documentation generation, Supabase Team with HIPAA add-on for backend, and Google Workspace for cloud sync (iCloud is unavailable in clinical mode — Apple does not offer a BAA for iCloud).
Same audio-deletion architecture as the consumer product. Same transcript-first design. Same premium-accuracy transcription. With a legal foundation that lets you actually use it for patient care.

MEAT-aware SOAP templates
Generate Subjective / Objective / Assessment / Plan documentation from your patient encounter. Each documented condition gets a MEAT analysis (Monitor / Evaluate / Assess / Treat) showing which elements are documented and which need elaboration before signing.
Evidence-linked diagnosis suggestions
ICD-10 code suggestions with V28 HCC mapping. Each suggestion shows the exact transcript span that supports it — so you can verify and edit before pasting the note into your EHR. Loqui doesn't auto-submit codes anywhere; the signed EHR note remains the audit-defensible documentation.
Workspace-only sync
Documentation syncs to Google Workspace (covered by Workspace BAA). Personal Drive and iCloud accounts are blocked in clinical mode — their consumer terms don't cover PHI.
Default template per user
Set SOAP as your default. Every recording during your clinic day becomes a SOAP note draft automatically. One tap to switch templates if a recording isn't a patient encounter.
HIPAA-grade security
Biometric idle lock, screenshot prevention on PHI views, encrypted local storage (SQLCipher), encrypted in transit (TLS 1.2+), encrypted at rest (AES-256), comprehensive audit logging that captures metadata without content.
Audio still gets deleted
The core privacy architecture doesn't change in clinical mode. Audio is destroyed once the transcript is written. Even your BAA doesn't make audio retention a good idea — less PHI on Loqui's infrastructure means less risk for everyone.
Loqui generates documentation drafts. The provider reviews, edits, and signs the final note in their EHR. The signed EHR note is the legally authoritative record. Loqui is a documentation aid, not a system of record.

The Clinical tier is targeted for the third or fourth quarter following Loqui’s general launch. Real BAAs take time. Real security audits take time. Real prompt validation against actual encounter transcripts takes time.
We’d rather launch a slower, correct product for clinicians than a fast, half-working one. The DOJ’s 2026 actions on unsupported diagnosis submissions make it clear that documentation mistakes carry real consequences. The tools that draft those documents need to be built carefully.
